QUESTIONNAIRE ON SARS

The purpose of this study is to gather information that will help us better understand how individuals are understanding, reacting and coping with Severe Acute Respiratory Syndrome (SARS). Understanding the psychological factors and responses to SARS may help to identify more effective intervention strategies to help people cope with the SARS threat. Potential risks include answering questions on a potentially upsetting topic (SARS), however you may benefit from this study by gaining insight into how you are dealing with this threat.

If you choose to participate in this study, you will be asked to participate in an on-line questionnaire on SARS. Completion of the questionnaire is entirely voluntary, will have no effect on your medical care, and that you can terminate your participation at any time. You have the right to refuse to participate or withdraw from participation without penalty of any kind.

All information is confidential and will be used for research purposes only. Anonymity is ensured, as your name will not appear on the questionnaire. This consent form will be removed before the records are processed. The data will be securely stored by the principal investigator.

In no way does this waive your legal rights nor release the investigators, sponsors, or involved institutions from their legal and professional responsibilities.
